2. Nicknames, abbreviations, or initials are not acceptable unless used as your legal name.
3. REQUIRED: Changes must be accompanied by evidence of current use of legal change. Acceptable documentation includes: marriage license, driver's license, court order, state identification card, social security card or passport.
4. If providing this documentation by mail, please send copy only.
5. Return your request to the Office of the Registrar with appropriate required documentation, or copy as directed.
